We currently have an opportunity for a Part-Time (0.4 FTE) Clinical Dietician at the Grove Long Term Care Home. Reporting to the Manager of Environmental and Food Services, the Clinical Dietician is responsible for completing comprehensive patient assessments and develop, monitor and revise nutrition care plans to address therapeutic dietary needs. The Clinical Dietician will provide an analysis of menus and approve all menus prior to implementation in the nursing home and assesses nutrition risk levels for all residents.

Duties include but are not limited to the following:

  • Complete in-depth intake assessments and determines / assigns level of nutritional risk per MOH Long Term Care Act requirements
  • Review all residents at a minimum quarterly and updates Nutrition Care Plans accordingly.
  • Provide nutrition care utilizing best practices, professional guidelines and Code of Ethics.
  • Participate as a member of an Interdisciplinary Team at Case Conferences and Annual Reviews, and as a member of various committees such as Wound Care, Accreditation, etc.
  • Provide nutritional education and counselling to residents, family and staff as required.
  • Review cycle menus planned for residents (regular, therapeutic, texture modified and snack) to ensure they meet nutrition requirements and menu planning guidelines
  • Provide approval whenever a new cycle menu is introduced
  • Contribute to continuous quality improvement of nutrition care program through regular service delivery audits, e.g. number of admission assessments completed within 21 days of admission as per standard, or high-risk residents assessed monthly as per standard.

Qualifications:

  • Member in good standing with the College of Dietitians of Ontario is required
  • Previous experience in Clinical Nutrition
  • Previous Long term Care experience
  • Excellent written and oral communication skills
  • Basic Computer skills.
  • Membership in Dietitians of Canada and Gerontology Practice Group preferred.

 

Hourly Wage Rate: $45.86 to $51.31
